Initiated in Summer 2014, GSC's Community Health Worker (CHW) reaches out to patients where they live with information and education about physical and mental wellness.

 

Our proactive CHW out-reach program involves visiting patients in their own homes with screenings for diabetes, hypertension and cholesterol; consultations on disease management; sharing information and education throughout the patient's support system on health, nutrition, wellness, and social concerns such as substance abuse
and teen pregnancy.

 

We maintain a reciprocal partnership with other local social service agencies.
