Community Health Worker (CHW)

Initiated in Summer 2014, GSC's new Community Health Worker (CHW) reaches out to patients where they live with information and education about physical and mental wellness.

Our proactive CHW out-reach program involves visiting patients in their own homes with screenings for diabetes, hypertension and cholesterol; consultations on disease management; sharing information and education throughout the patient's support system on health, nutrition, wellness, and social concerns such as substance abuse and teen pregnancy.

We maintain a reciprocal partnership with other local social service agencies.
